INSTRUCTIONS FOR THE TRANSFER OF SECURITIES VIA DEPOSITORY TRUST COMPANY (DTC)
PLEASE FOLLOW THE INSTRUCTIONS BELOW FOR A SUCCESSFUL STOCK TRANSFER.
Notify your bank or broker that you would like to transfer shares as a charitable gift to Historic St. Luke’s Restoration, Inc. Tax ID number 54-0629148. Identify how many shares you wish to contribute, as well as their type. (Please note: Your bank or broker may require instructions in writing with a signature guarantee.)
Historic St. Luke’s Restoration, Inc. has two accounts available to receive your gift. Please choose one and provide the corresponding transfer instructions as noted below.

PLEASE FOLLOW THE INSTRUCTIONS BELOW FOR A SUCCESSFUL STOCK TRANSFER.
- Please call Todd Ballance at 757-357-3367 or email [email protected] and state your intention to donate stock held in certificate form to Historic St. Luke’s Restoration. Our financial advisor will then contact you.
- After receiving instructions from our financial advisor, please send, in a single envelope, the unendorsed certificates (please DO NOT sign or otherwise endorse the certificates) by first-class mail to:

- Please include a cover letter with the certificates. This letter should state your intention to make a gift of stock to Historic St. Luke’s Restoration, Inc. All owners listed on the certificates must sign the gift letter exactly as their names are shown on the certificates
- In a separate envelope, mail to the same address sufficient powers to complete the gift- one stock power per individual certificate. (You should receive the appropriate paperwork from our financial advisor.) The stock powers must be left blank and be signed by the owners listed on the stock certificate exactly as shown on the certificate. Lastly, the signatures on the stock powers must have a signature guarantee from a bank or stockbroker.
